Death Records
Before your request for a certified copy of a death record can be considered, you must specify your eligibility to obtain it. Illinois law states that copies of death records may be issued only as listed below:
Upon the specific written request for a certified copy by a person, or his duly authorized agent, having a personal or property right interest in the record. ALL information marked “REQUIRED” on the application must be completed in order for us to process your request. Failure to complete all “REQUIRED” information of the application will cause a delay in receiving the copy or copies of the death certificate.
Birth Records
Illinois law states that only a parent listed on the birth record or legal guardian can obtain a copy of a child’s birth record. The child, if over 18, can request a copy of his/her certified birth certificate. ALL information marked “REQUIRED” on the application must be completed in order for us to process your request. Failure to complete all “REQUIRED” information on the application will cause a delay in receiving the copy or copies of the birth certificate.
Obtaining Copies
If you believe you are eligible to request a Birth or Death record, please complete the application and mail it with a picture ID and a check or money order for the required amount to the Vermilion County Health Department ATTN: Vital Records. You can, also, fax the application and a copy of the picture ID to 217-431-7483 and pay by debit or credit card by using Gov.Pay.Net. Gov.Pay.Net does charge a fee for each transaction.
